It's a common mistake, after installing failsafe file many people reboot, turn car off, software message at the end of failsafe install instead of saying "No reboot required" should say "Don't reboot, continue with reinstall file".
In my bench test sometimes it will continue to work but eventually after a few reboots it would get the black screen of death.

Lets start from the beginning what firmware you had and what are you trying to update to and post a clear picture of your CH341A programmer with test clip attached to ZIF connector.
