I recently had to install a new clutch into my 2008 Mazda 3 hatchback 2.3 l and the clutch ended up disintegrating the throwout bearing as most of you will know that there's constant problems with the clutch throwout bearings in the Mazda 3s at least in my experience cuz I can't keep my foot completely off the pedal LOL
So I destroyed the fork that moves the throat bearing. As I was looking for a new fork I called Mazda dealership and they actually have a new upgraded clutch throw out bearing fork, throw up bearing fork spring, throw out bearing, pivot ball! It has made a tremendous difference in the reliability of the throwout bearing as well as seems to have eliminated the problem with the throwout bearing staying on the throwout bearing fork under heavy acceleration and or shifting. Not sure if there's another post for this yet but I'm assuming there is but if not hopefully some of you will benefit from calling the dealership and upgrading the parts I just spoke of. Thanks

Yes ... Mazda Upgraded the throw out plus other parts around it.

I had the same issue with throw out failing at about 230K KMS
It was not a recall for my model year.

I would suggest that you replace it when next replacing the clutch
